Cardiac Enhancement Center
For people living with chronic heart failure, the challenges and lifestyle accommodations can be major. The Cardiac Enhancement Center at Evergreen offers ongoing support and management, and also helps coordinate care between your primary care physician and your heart specialist. Extensive education for patients and their support is provided. Weekly visits and phone follow-ups are at the core of this service, which has been recognized by Qualis, the state's quality-improvement organization. Rehabilitation
The Sound Heart cardiac rehabilitation program helps patients who have experienced a heart attack, coronary bypass surgery, cardiology procedures (such as angioplasty) and/or have congestive heart failure. The typical program lasts six to 12 weeks.
